What is Dienogest?

Dienogest is a progestin treatment used for endometriosis, a chronic condition in which endometrial tissue grows outside the uterus. This growth causes inflammation, resulting in severe pain and heavy bleeding. While there’s no cure for endometriosis, medications like Dienogest can help manage symptoms. They’re typically taken daily long-term.

Dienogest can suppress ovulation, which helps relieve pain associated with endometriosis.

How does Dienogest work?

Dienogest works by creating a low-oestrogen, high-progestin environment. Endometriosis can cause intense pain during ovulation due to inflammation, scar tissue, and hormonal imbalances. By lowering oestrogen levels, it can help prevent ovulation, which can reduce pain.[1] Additionally, higher progestin levels further suppress oestrogen production, which decreases the growth of endometrial tissue and reduces inflammation. This can help relieve pain related to endometriosis.[2]

What doses of Dienogest are there?

Dienogest tablets come in a dose of 2mg. They’re typically used on a daily basis for long-term treatment.

How to take Dienogest

  • Take one tablet daily with water.
  • It should be taken at the same time each day.
  • Continue taking Dienogest even during menstruation.

Don’t take Dienogest with grapefruit, as this increases the amount of Dienogest in your blood, which can increase the risk of side effects.

How long does Dienogest take to work?

Dienogest is absorbed quickly, reaching peak concentration within 1.5 hours.[1] But it might take a few weeks or months for your symptoms to improve. This is because it takes time for your oestrogen levels to decrease and progestin levels to increase.

Endometriosis pain is also typically at its worst during ovulation, so you might notice greater improvement in your symptoms at this time.

What should I do if I forget to take a dose of Dienogest?

If you miss a tablet, take it as soon as you remember. But don’t take a double dose to make up for a missed one. Forgetting a dose of Dienogest can make it less effective, so it’s important you take it daily around the same time.

Will I get side effects with Dienogest?

Answer:
Some people have reported side effects when taking Dienogest, such as:

  • Weight gain

  • Low mood

  • Sleep problems

  • Change in libido

  • Headache

  • Nausea

How well does Dienogest work for endometriosis?

Answer:
In one study, women using Dienogest reported a reduction in pain levels, with 55.2% improvement in the 2mg group and 68.6% in the 4mg group. The study noted that as treatment continued, pain improvement was more pronounced. Many patients found Dienogest tolerable, with very few discontinuing the treatment.[1]

Another study supported these findings, showing that 52.9% of patients treated with Dienogest experienced symptom improvement, compared to only 22.9% in the placebo group by week 12.[1]

Overall, Dienogest is an effective and safe treatment for endometriosis.

Does Dienogest work as a contraceptive?

Answer:
No, Dienogest doesn’t work as a contraceptive. Although it contains a progestin and works similarly to a mini-pill (progesterone-only pill), it doesn't stop ovulation completely in all women. So there's a chance you could still get pregnant.

While taking Dienogest, you should use condoms or non-hormonal contraceptives, such as an IUD, to prevent pregnancy. You should avoid using hormonal contraceptives while using Dienogest, as they can increase the risk of side effects.

Will I get symptoms after stopping Dienogest?

Answer:
How quickly symptoms return (if they do at all) can depend on your individual response to the drug.

Symptoms can return after stopping Dienogest as hormone levels return to normal. But they don’t usually come back straight away. It can take a few weeks for your hormone levels to regulate and for symptoms to reappear.

Some people, on the other hand, notice symptoms returning gradually, while others may have a longer symptom-free period after stopping treatment.

Will I get periods while on Dienogest?

Answer:
While it’s possible to have periods while taking Dienogest, this isn’t common. Dienogest prevents ovulation, so there’s no uterine lining to shed, resulting in the absence of periods.

It’s common to experience irregular bleeding or changes in bleeding patterns, but this typically tends to be light or infrequent. If you experience heavy or continuous bleeding, let your clinician know.[2]

After you stop Dienogest, your period should return within a few weeks.[1]

Can I get pregnant while taking Dienogest?

Answer:
No, you should take precautions to avoid becoming pregnant while taking Dienogest, such as using barrier contraception. This is because Dienogest increases the risk of ectopic pregnancy, where the embryo develops outside the womb.

If you become pregnant while taking Dienogest, contact your doctor immediately.
