Pivmecillinam is also available under the brand name Selexid. Both contain the same amount of the active ingredient and work in the same way, but the brand name is usually a bit more expensive.
Pivmecillinam is recommended for lower UTIs if Nitrofurantoin (MacroBID) or Trimethoprim aren’t suitable, or haven’t worked. Evidence shows that all three options are similarly effective at clearing uncomplicated infections.[1] However, your clinician might recommend Pivmecillinam for you if:
- You can’t take Nitrofurantoin (due to kidney problems or side effects, for example)
- Local bacterial resistance rates to Trimethoprim are high in your area
- Urine tests show that other standard antibiotics might not be as effective for your specific infection.
If you’re pregnant and have lower UTI symptoms, then you may be prescribed amoxicillin 500mg three times a day for 7 days if nitrofurantoin (the first choice treatment) taken for at least 48 hours didn't work. Another option is cefalexin 500mg twice a day for 7 days. But Pivmecillinam is also suitable during pregnancy, if the other treatments are not available or if you’re allergic to them.
The most suitable treatment for your infection will depend on your unique situation. Your clinician might suggest taking a urine test initially to identify the specific bacteria causing the infection. This helps them choose the most suitable treatment for you, which may differ from the antibiotics mentioned earlier.
