When it comes to Wegovy and kidney disease, everyone’s circumstances are different. It’s really important to speak to a medical professional before starting any new medication if you have problems with kidney function, to make sure it’s suitable for you. But there is evidence that the health benefits of Wegovy could improve your kidney function as well as helping with weight loss.

Wegovy is a type of medicine called a GLP-1 agonist that mimics the natural GLP-1 hormone in your body. It affects the parts of your brain that regulate your appetite to help you feel fuller for longer after eating and less hungry in general. Wegovy also encourages the release of insulin when your blood glucose levels are high (like after eating), keeping your blood sugar levels under control.

By helping you reduce your calorie intake to encourage weight loss and managing your blood sugar levels, Wegovy can take strain off your kidneys and may improve your kidney health.

Is Wegovy safe for people with kidney disease?

Generally, yes – Wegovy is safe for people with kidney disease. However, it’s really important that you speak with a clinician before you start any new medication, so they can make sure it’s the right option for you.

In fact, there’s some evidence that Wegovy could help protect (and even benefit) your kidney function. A FLOW trial suggested semaglutide, the active ingredient in Wegovy, could help lower the risk of kidney disease progressing by 24% in people who have both type 2 diabetes and chronic kidney disease. Since then, more recent research has reported that semaglutide can help protect kidney function in those who have overweight or obesity with existing cardiovascular disease. One study reported that for people with obesity, losing weight resulted in decreased levels of protein in urine – with urinary protein being an indicator that your kidneys aren’t working properly.

How does taking Wegovy help improve kidney health?

Losing weight can help kidney function

There’s a lot of scientific evidence that points to a strong link between obesity and the risk of developing or progressing chronic kidney disease (CKD). For example, a study looked into the prevalence of chronic kidney disease in different groups of people. These groups were: male, female, those younger than 43, those older than 44, those who have obesity, those who don’t have obesity, and those with a family history of renal disease. This study found that CKD was most prevalent in the group with obesity, affecting one-third (32.9%) of participants. High BMI and obesity are also associated with CKD progressing faster in those who already have issues with kidney function, too.

The good news is that it doesn't take a huge change in your weight to help protect your kidney function and overall health. In fact, losing just 10 pounds could help with high blood pressure and reduce your risk of developing diabetes or heart disease.

Blood sugar levels can affect the kidneys

Your blood sugar levels and kidney health are really closely linked. If your blood sugars stay too high for too long, they can damage your blood vessels in your kidneys by causing them to harden and narrow. When the blood vessels in your kidneys become damaged, they aren’t able to filter your blood as well as they should, which can cause waste products to build up in your bloodstream.

The active ingredient in Wegovy, semaglutide, was originally licensed as a medicine to manage blood sugar levels in people with type 2 diabetes. When your blood sugars are high, your body will normally produce insulin to reduce the levels of glucose in your blood. Semaglutide stimulates your body’s natural release of insulin when your blood sugars are high, so you make more insulin when you need it. It can also help to reduce how much sugar is made and sent into your bloodstream by your liver, further helping to keep your blood glucose levels in check. This should protect your kidney function from issues caused by high blood sugar levels.

Wegovy can reduce inflammation

While Wegovy is only licensed as a weight loss medication, recent evidence suggests it may also have anti-inflammatory properties. Inflammation is a key factor in lots of conditions and complications related to diabetes, which means Wegovy could offer extra protection for kidney function, cardiovascular health, and liver health than first thought. However, evidence for the specific mechanisms involved is still limited – and more trials and studies are needed to examine the extent of Wegovy's effect on inflammation.

What can be done to improve overall kidney health?

While there is some evidence that Wegovy can help protect your kidney function, it isn’t designed to reduce the risk of kidney disease; you won’t be prescribed Wegovy to treat kidney problems if you aren’t living with obesity or overweight. However, there are lots of other things you can try to protect or improve your kidney health.

  • Eat a balanced, healthy diet. This can help reduce the risk of kidney disease by keeping your blood pressure and cholesterol in check.
  • Limit how much alcohol you drink. Heavy drinking can increase your blood pressure and cholesterol, which can in turn damage your kidneys. You should avoid drinking more than 14 units of alcohol a week regularly, and spread the alcohol you drink over three or more days if possible.
  • Get active. Exercising regularly can help to lower your blood pressure and protect your kidney function. You should aim to get at least 150 minutes of aerobic activity each week (like cycling, fast walking, or playing a sport).
  • Manage other conditions. Health conditions like diabetes, high blood pressure, high cholesterol, and heart disease can all cause damage to your kidney function. Keeping on top of these with prescription medicines and lifestyle changes is really important for protecting your kidneys.
  • Limit non-steroidal anti-inflammatory painkillers (NSAIDs). Painkillers like aspirin and ibuprofen can cause kidney disease if you take too many, or take them for a long period of time. Try using alternative painkillers where possible – but if you need to use NSAIDs, make sure you follow the instructions fully to limit the risk of any side effects.
  • Quit smoking. Smoking can damage your kidneys by limiting the blood that flows into them, and increasing your blood pressure. It can also increase your risk of certain kidney cancers.
  • Lose weight. If you have obesity or overweight, losing weight can take some of the strain off your kidneys. Research shows that for people with obesity, losing weight results in decreased levels of protein in urine (with urinary protein being an indicator that your kidneys aren’t working properly).

But if you have overweight or obesity as well as kidney disease, speak to a doctor to see if Wegovy could be right for you. There’s a lot of scientific data that shows losing weight can take some of the strain off your kidneys and improve your overall health.
