HRT gel, or oestrogen gel, is a topical treatment that you apply to your skin once a day. The oestrogen is absorbed through the skin and works to improve the symptoms of menopause, as well as help to maintain your long-term health. It can be particularly useful for you if you’ve had a hysterectomy but if you haven’t you can still use it, you’ll just need to take a separate form of progesterone as well.
There are two forms of oestrogen gel: Oestrogel, which comes in the form of a pump, and Sandrena, which comes in sachets. Sandrena is a slightly stronger dose than Oestrogel, and because it comes in sachets it can make it harder to tailor your dose than with the Oestrogel pump pack.
When your body goes through the menopause, it starts producing less of the sex hormone, oestrogen. This decrease in oestrogen levels can cause a range of symptoms associated with menopause, such as hot flushes, mood changes, vaginal dryness and night sweats, which can vary in severity from person to person. (Some women experience barely any symptoms of menopause, whereas for others they can be debilitating.) Decreased oestrogen levels can also increase the risk of health issues over time, such as lower bone density (osteoporosis).
HRT gel contains a synthetic version of the hormone oestrogen. When you apply it to your skin, it’s absorbed into your body to replace the drop-off in oestrogen levels caused by the menopause. This helps to ease the symptoms of menopause, as well as reduce the risk of long-term problems like osteoporosis.
Taking oestrogen for menopause without progesterone has been shown to cause an increased risk of womb cancer, so if you haven’t had a hysterectomy you’ll need to take a separate progestin alongside your HRT gel to reduce this risk.
Oestrogel comes in a dose of 750mcg per actuation (pump). The amount of pumps you use will be dependent on the strength of your symptoms.
Sandrena gel, on the other hand, comes in sachets of either 0.5mg (500mcg) or 1mg.
You may be prescribed between 1-4 pumps per day of Oestrogel for your symptoms, which is a maximum of 3mg per day, whereas Sandrena gel is usually prescribed in doses ranging from 0.5mg to 1.5mg. Your clinician will want to prescribe the lowest possible dose of treatment to help your symptoms (and it’ll help your treatment last longer, too), so although the highest dose of HRT gel is 3mg, you’ll likely only use this dose if your symptoms are severe.

HRT gel should be applied to your skin for it to be absorbed into your bloodstream. The leaflet that comes with your treatment will give you precise instructions on how to use it but, generally speaking:
It usually takes at least three weeks for you to start feeling the effects of any HRT, and oestrogen gel is no different. For some women it can take up to three months for them to start feeling the full benefit of it. If you’re still struggling with symptoms after 4-6 months you may need to try an alternative form of HRT.
It’s recommended that you either apply your HRT gel in the morning or the evening, so whenever suits you best, really. Do bear in mind, though, that you should avoid close skin contact with anyone else for at least an hour after applying the gel. So if you share a bed, you should make sure that you apply the gel at least an hour before doing so.

Oestrogen gel has a few benefits. For starters, it doesn’t carry some of the same risks that you get with HRT tablets, such as indigestion and blood clots. You might also prefer it to having to apply a patch, especially if you’re concerned that the patch might fall off or become loose as you’re going about your daily life.
Oestrogen gel has also been shown to be just as effective as other forms of oestrogen-only HRT.
Yes, there aren’t any issues with switching HRT (so long as you’re following the advice of your clinician). If you’re taking combined HRT, though, you’ll need to make sure that you continue to take a separate progesterone alongside your oestrogen gel.
It depends. If you’re taking an oestrogen-only HRT but still have your womb, then you’ll need to take a progestin alongside this. However you shouldn’t really use two different HRTs that contain oestrogen, as you might be using too high of a dose.
The only real exception to this rule could be if you’re still experiencing vaginal symptoms despite taking HRT. In this instance, you may be prescribed a vaginal oestrogen alongside your HRT to improve your vaginal symptoms, without having to take a higher dose of HRT.

All medications can cause side effects, and HRT is no exception to this rule. If you’re concerned about side effects, though, evidence suggests that transdermal HRT (such as gels or patches) can cause fewer side effects than oral HRT (tablets).

Yes, according to UK government advice, you can take HRT gel in your hand luggage when travelling abroad. Even if your treatment is over the 100ml limit, you can still take it as long as you have a clearly labelled prescription with your name on it, or a letter from your doctor.
No, you can’t buy oestrogen gel over the counter, as it’s a prescription-only medication. This is because it’s not safe for everyone to use, and using it outside of its licence or without the approval of your clinician can carry risks.
At the time of writing there’s only one form of HRT that’s available without a prescription, which is Gina - a vaginal oestrogen treatment. This form of HRT isn’t a ‘full-body’ HRT, so it’ll only treat the vaginal symptoms of menopause, such as dryness. This form of HRT doesn’t carry the same risks as other HRT though, so you can take it without a progestin, even if you haven’t had a hysterectomy.
